THERAPEUTIC INDICATIONS: ACORTIZ is a diuretic of the thiazide family is indicated for:
Hypertension: As monotherapy or in combination to increase the effect of other antihypertensive agents when it comes to more severe forms of hypertension.
Edema Associated with congestive heart failure, liver cirrhosis and corticosteroids and estrogenoterapia.ACORTIZ is also useful in edema associated with renal dysfunction such as nephrotic syndrome, glomerulonephritis and chronic renal failure.
Pharmacokinetics IN HUMANS: Hydrochlorothiazide is absorbed orally in 65-75% of the administered dose and plasma half-life (t ½) is 2.5 hours.
Terminal half-life is 9 to 14 hours on average. Hydrochlorothiazide is not metabolized and is excreted largely unchanged through the kidney.
The diuretic action of hydrochlorothiazide starts two hours after oral administration and peak effect at 4 hours, the action persists for 6 to 12 hours.
Hydrochlorothiazide is known to affect the mechanism of tubular reabsorption of electrolytes. A maximum therapeutic dose thiazide all increase the excretion of sodium, chloride and water at the distal convoluted tubule in equivalent amounts.
The excretion of potassium and magnesium is directly proportional to the administered dose, while increased calcium resorption.
CONTRAINDICATIONS: The use of ACORTIZ is contraindicated in patients with anuria and hypersensitive to the active substance or formulation components and other sulfonamide-derived drugs.
PRECAUTIONS: In patients receiving higher doses of thiazides, there may be cases of hyperuricemia or frank gout. A latent diabetes may become manifest with treatment based on thiazides.
Higher-dose diuretics may precipitate azotemia in patients with renal insufficiency.
Use in Pregnancy and Lactation: Not recommended for healthy pregnant or edema because it exposes mother and fetus to unnecessary risk. Diuretics do not prevent development of toxemia of pregnancy and there is no conclusive evidence that they are useful in treatment.
Thiazides are indicated in the edema of pregnancy only when it happens for pathological causes. Thiazides cross the placental barrier and appear in cord blood, the risk of neonatal jaundice, thrombocytopenia, and other side effects.
ADVERSE REACTIONS:
Gastrointestinal: Anorexia, gastric irritation, nausea, vomiting, cramps, diarrhea and, very occasionally, pancreatitis, and sialadenitis.
CNS: Dizziness, vertigo, paresthesias, and headaches.
Hematologic: Leukopenia, agranulocytosis, thrombocytopenia and aplastic anemia.
Cardiovascular: Orthostatic hypotension.
Hypersensitivity: Purpura, photosensitivity, rash, hives, fever, and anaphylactic reactions.
Other: Hyperglycemia, glycosuria, hyperuricemia, muscle spasm, weakness, restlessness, and transient blurred vision.
DRUG INTERACTIONS AND OTHER GENDER: drug interaction may occur when administered ACORTIZ jointly with the following drugs:
Alcohol, barbiturates or narcotics: There may be increased orthostatic hypotension.
Pressor amines: May decrease the response to pressor amines.
Amphotericin B, corticosteroids or ACTH: electrolyte imbalance may increase, particularly hypokalemia.
Oral anticoagulants: May decrease anticoagulant effects.
Inflammatory agents: These can reduce the diuretic effect.
Cholestyramine and colestipol: Slow ACORTIZ absorption.
Cardiac glycosides: There is possibility of increased toxicity associated with hypokalemia digital.
Oral hypoglycaemic agents and insulin: May require dose adjustment of antidiabetic agents.
Lithium: Diuretic agents reduce the renal clearance of lithium and increase the risk of toxicity.
Medicines for gout: May increase serum uric acid level.
Other antihypertensive drugs: additive effect or potencializante of its effects.
Probenecid or sulfonilpirazona: On the effect of thiazides hyperuricaemic diminished its effectiveness can be used.
Skeletal muscle relaxants, nondepolarizing: Increase likely response to the muscle relaxant.
CHANGES IN RESULTS OF LABORATORY TESTS: hyperkalemia may occur, increases in uric acid and creatinine.

PRECAUTIONS IN RELATION TO EFFECTS OF CARCINOGENESIS, MUTAGENESIS, Impairment of Fertility: Not enough information on the effect of hydrochlorothiazide on fertility, teratogenicity, mutagenicity or carcinogenicity in humans.
DOSAGE AND ADMINISTRATION: ACORTIZ is administered orally. The applied dose should be individually according to patient response.

Very few patients in both therapies require sustained high doses.
REPRESENTATIONS AND MANAGEMENT Overdosage: In addition to the expected diuresis, overdosage of ACORTIZ can produce varying degrees of lethargy which may progress to coma within a few hours with minimal depression of respiration and cardiovascular function and without evidence changes in serum electrolytes or dehydration.
In addition to gastric lavage and supportive therapy for stupor or coma, may be necessary to treat gastrointestinal effects. Has not been established clearly the degree to which hydrochlorothiazide is removed by hemodialysis.
